VNS Services
We offer many services through our Acute Care Program (ACP), through
our Chronic Care Program (CCP), or in many cases through both.
For other services, please visit our associated home care organization
at Community Health and Home Care (CHHC).
- Skilled Nursing (ACP or CCP)
- Experienced registered professional nurses provide physician ordered
treatments to patient at their home. Examples of skilled nursing services
include intravenous therapy, pain management, IV or oral administration
of medicine, care of central infusion lines, blood drawing, urinary
catheter care, disease management, and case management in dealing with
the medical needs of the patient.
Home Health Aide (ACP or CCP)
- Paraprofessionals who are certified to perform health related tasks
under nursing supervision. This includes personal care, light housekeeping,
physical therapy exercises under the supervision of a physical therapist,
and meal preparation.
Occupational Therapy (ACP or CCP)
- Occupational therapy treatments are designed to improve or restore
functions that have been affected by injury, illness, or surgery. With
the use of exercises, activities and interventions an Occupational Therapist
can help people regain health and function that leads to accomplishment,
mastery, and a sense of purpose and meaning in life.
Physical
Therapy (ACP or CCP)
- Helps patients to strengthen muscles, improve the use of their joints
(knees, ankles, shoulders, hands), decrease pain and become comfortable.
Improves mobility, safety and independence at home.
Medical Social Services (ACP or CCP)
- Counseling provided by a certified social worker to assist the patient
and family in dealing with the problems related to the disease or disability.
Social work may also be provided to assist patients and their families
to access and community services that may help maintain safety and well-being.
Nutritional Services (ACP or CCP)
- A registered dietician is available to assess dietary needs, assist
in developing a nutritional plan tailored to those needs, and provide
instruction on how to maintain a dietary program that is appropriate.
Speech Language Pathology (ACP or CCP)
- Works with swallowing problems, developing exercises to improve communication,
designing and implementing exercises to strengthen oral muscles and
function in nervous disorders. Improves or facilitates swallowing, oral
motor function, speech production and cognitive function.
Personal Care Aide (CCP only)
- Certified paraprofessionals assist the patients with personal care
and light household maintenance.
Audiology (CCP only)
- Evaluation provided of a person's ability to receive verbal communication
and recommending appliances to improve the hearing of a person with
a hearing impairment.
Respiratory Therapy (CCP only)
- Services related to a patient's ability to exchange oxygen and breathe
more effectively. Respiratory therapists can administer inhaled medications,
evaluate for the need for oxygen therapy, and encourage breathing exercises
that improve oxygen intake.
Housekeeper/Homemaker (CCP only)
- Assists with routine housekeeping and companion tasks for clients
in their place of residence. Housekeeper/Homemakers do household tasks
for persons at home because of illness, incapacity, or the absence of
a caretaker relative. Service can include caring for family clothing,
including light washing, ironing, mending and sorting clothes; cleaning,
dusting, washing dishes and performing a variety of other housekeeping
tasks to keep the home in a sanitary and livable condition; running
errands, and attending activities with the client.
